Submit referral confirmation
Have the referral completed and signed by your doctor and submit it online. Alternatively, send it by post to: CSS, Postfach 2550, 6002 Luzern.
Important to know: If you're insured under the family doctor model and your doctor doesn't digitally transfer the referral to CSS, you must confirm the referral.
